头颈部恶性外毛根鞘癌的临床特点及预后分析

目的：探讨恶性外毛根鞘癌(TLC)的临床诊治及其预后。方法：回顾性分析26例TLC的临床资料，其中单纯手术24例，手术+化疗2例。采用SPSS18.0行生存分析和对数秩检验。结果：22例位于头皮，4例位于面颈部。溃疡型7例，角化结节型19例。24例无复发，2例术后复发并伴淋巴结及远处转移。于中山大学肿瘤防治中心头颈科初治2例，非规范术后24例(肿瘤残留率66.6%)。5年生存率为86.1%，手术切缘、淋巴结转移和肿瘤是否残留均是影响预后的因素。结论：TLC是低度恶性肿瘤预后较好，TLC的治疗首选手术，建议安全边界为1.5 cm。 Objective To study the clinical characteristics，the surgical treaments and prognostic factors of trichilemmal caricinoma.Method The data of twenty-six cases with trichilemmal caricinoma was retrospectively reviewed and analyzed.The treatment modalities included surgery alone in twenty-four cases and surgery plus chemotherapy in two cases.Statistic analysis was performed usingsofeware SPSS 18.0.Results 22 cases located in the scalp，4 cases located in the face and neck.7 cases of ulcer，19 cases of nodular keratosis.24 cases without recurrence，2 cases of recurrence and lymph node and distant metastasis.2 cases of initial treatment in the department of head and neck，Sun Yat-sen cancer center，24 cases of non-standardized surgery(Residual tumor rate was 66.6%).The 5-years overall survival of the whole group was 86.1%.surgcial margin，neck lymph node metastasis and residual tumor were significantly correlated with the prognosis.Conclusion TLC has a good prognosis.Complete surgical excision is the first choice of treatment.Surgical margins of 1.5 cm seem to be safe.
