目的：探讨心力衰竭患者快速检测全血N末端B型脑钠肽前体（ NT-proBNP）的临床价值。方法：选取心内科收治的心力衰竭患者78例作为观察组，选取同期非心血管病患者15例作为对照组，入院0.5 h内用Nano-Checker710免疫层析检测仪完成床边快速测定全血NT-proBNP浓度。观察组患者出院后随访3个月，记录各组因不良心血管事件而再次住院或死亡的患者例数。结果：观察组按NYHA心功能分级，随着心功能分级逐渐加重，NT-proBNP含量呈明显上升趋势，与心力衰竭的严重程度呈正比。观察组患者出院后随访3个月，Ⅲ级组因不良心血管事件而再次住院2例，Ⅳ级组因不良心血管事件而再次住院6例、死亡2例。结论：全血NT-proBNP水平与心力衰竭的严重程度存在密切的关系，可作为反映心功能状态的良好的客观指标。床边快速检测全血NT-proBNP浓度可对心力衰竭做出及时准确的诊断性评估，并对近期预后判定具有一定的价值。 Objective To study the clinical value of rapid bedside detection of whole blood N-terminal B-type natriuretic peptide ( NT-proBNP)in patients with heart failure( HF). Methods Select 78 patients with heart failure admitted to Cardiology department of our hospital as the observation group,select 15 cases of non-cardiovascular disease in the same period as a control group,within 0. 5 h using Nano-Checker710 rapid determination of the detector completes the bedside NT-proBNP concentrations in whole blood. The patients were followed up 3 months after discharge,record the number of cases in each group due to adverse cardiovascular events hospitalized again or death . Results Observation group by NYHA class,with the cardiac function gradually worsened,NT-proBNP levels showed a clear up-ward trend,and is proportional to the severity of heart failure. The patients were followed up 3 months after discharge,III level group due to adverse cardiovascular events readmission two cases,IV level group due to adverse cardiovascular events and readmissions 6 cases,2 deaths. Conclusion There is a close relationship between cardiac function and NT-proBNP levels in whole blood . NT-proBNP levels can be used as a good reflection of objective indicators. Rapid bedside whole blood NT-proBNP concentrations in heart failure to make timely and accurate diagnostic evaluation,and also has a certain value on the recent prognosis.
